Objectifs: Examiner l’association entre la prise de poids maternelle et un poids de naissance inférieur à 3000g, ou supérieur ou égal à 4000g, pour les femmes en sous-poids (indice de masse corporelle IMC inférieures à 19.8 kg/m2), de poids normal (IMC 19.8–26.0 kg/m2), en surpoids (IMC 26.1–29.0 kg/m2) et obèses (IMC supérieure à 29.0 kg/m2), en étudiant l’utilisation au Danemark des recommandations de l’Institut of Medicine (IOM) américain

Méthode: les données sur 2248 femmes avec des grossesse non gémellaires à terme ont été analysées. Les relations entre la prise de poids et le risque de naissance à moins de 1000g ou à 4000g et plus ont été examinées pour les 4 groupes d’IMC, et l’utilisation des recommandations de l’IOM au Danemark a été testée par analyse de régression logistique.

Résultats: Une relation inverse a été trouvée entre la prise de poids maternelle et la proportin de bébés nés à moins de 3000g. Les naissances à 4000g ou plus sont plus augmentent avec l’augmentation de la prise de poids pour les femmes en sous-poids ou de poids normal, mais l’association est moins évidente pour les femmes obèses et en surpoids. Les femmes en sous poids semblent avoir intérêt à prendre plus de poids que ce que recommande l’IOM, car le rapport des cotes (odds ratio (OR)) d’une naissance inférieure à 3,000 g est de 0.3 (intervalle de confiance (CI) 95% 0.1–0.9) et l’OR est de 1.7 pour les poids de naissance supérieurs ou égaux à 4,000 g (95% CI 0.8–3.6). Les femmes de poids normal ont un risque accru de naissance inférieure à 300g (OR 2.4, 95% CI 1.5–3.7) si la prise de poids est inférieure aux recommandations, et l’OR de poids de naissance supérieur ou égal à 4000g est de 1.9 (95% CI 1.5–2.5) lorsque les femmes prennent plus que le poids recommandé.

Conclusion: les recommandations de l’IOM peuvent être utilisées au Danemarque, bien que les limites supérieures de prise de poids pour les femmes en sous-poids devraient être revues à la hausse.

Abstract (English)  :

OBJECTIVE: To investigate the association between maternal weight gain and birth weight less than 3,000 g and greater than or equal to 4,000 g in underweight (body mass index [BMI] less than 19.8 kg/m2), normal weight (BMI 19.8–26.0 kg/m2), overweight (BMI 26.1–29.0 kg/m2), and obese (BMI greater than 29.0 kg/m2) women, with emphasis on the use of the American Institute of Medicine (IOM) recommendations in Denmark.

METHODS: We analyzed data from 2,248 women with singleton, term pregnancies. The relationship between weight gain and risk of birth weight less than 3,000 g and greater than or equal to 4,000 g was examined in the four BMI groups, and use of IOM recommendations was tested by logistic regression analyses.

RESULTS: We found an inverse relationship between maternal weight gain and the proportion of infants with a birth weight less than 3,000 g. Birth weight greater than or equal to 4,000 g increased with an increasing weight gain in underweight and normal-weight women, but the association was less apparent in overweight and obese women. Underweight women seemed to benefit from gaining more weight than recommended by the IOM, because the odds ratio (OR) of birth weight less than 3,000 g was 0.3 (95% confidence interval [CI] 0.1–0.9) and the OR was 1.7 for birthweight greater than or equal to 4,000 g (95% CI 0.8–3.6). The normal-weight women had an increased risk of birth weight less than 3,000 g (OR 2.4, 95% CI 1.5–3.7) if weight gain was below the recommended range, and the OR of birth weight greater than or equal to 4,000 g was 1.9 (95% CI 1.5–2.5) when the women gained more than recommended.

CONCLUSION: The IOM recommendations may provide a basis for Danish recommendations to pregnant women, although the upper recommended limit for underweight women may have to be increased.

Il en résulte que les recommandations américaines peuvent s’appliquer aux femmes de poids normal, en surpoids et obèses ; mais pour les femmes en sous-poids, en comparant les risques de bébés trop petits avec ceux de bébé trop gros, l’étude trouve que la limite de prise de poids maximum devrait être relevée.

Argument (English):

A Danish study on the risks of babies who are too small (less than 3kg) or too big (4kg and over) according to the maternal weight gain for each category of body mass index (BMI), compared to American recommendations. As a result, American recommendations can be applied to women of normal weight, overweight and obese; but for underweight women, comparing the risks of too small babies with those of overweight babies, the study finds that the limit of maximum weight gain should be raised.
