Section 2: Important or emerging areas of research

Evidence indicates that one significant cause of a woman’s perception of birth as traumatic is the actions or inactions of maternity staff, which can result in care being experienced as dehumanising, disrespectful or uncaring (Elmir, Schmied,
Wilkes, & Jackson, 2010; Goldbort, 2009).

Professionals’manner and communication can significantly affect women’s feelings of control during their delivery (Salter,2009) and their ability to make informed decisions (Eliasson, Kainz, & Von Post,2008). Yet choice, information and involvement in decisions are potentially protective against a traumatic birth experience (Goodall, McVittie, & Magill, 2009).

Therefore, professionals need to understand that childbirth can be traumatic for
women (Elmir et al.,2010); acknowledge the role they may play and recognise the
signs of psychological trauma (Beck,2004). There is also evidence that increased
empathy in staff can increase their own risk of developing PTSD symptoms after witnessing traumatic childbirth events (Sheen, Spiby, & Slade,2014). This means
that clinicians need to manage the difficult boundary between recognising and sup-
porting postnatal women with symptoms of trauma, while simultaneously maintaining their own professional boundaries in order to protect themselves**

Enquête à partir d’un questionnaire internet, destiné à des femmes qui considèrent avoir eu un accouchement difficile ou traumatique. Recrutement via internet: site Birth Trauma Association http://www.birthtraumaassociation.org.uk/, groupes Yahoo et Facebook en 2008. 675 réponses.
Les questions consistaient en:
- échelle de diagnostic du PTSD adaptée. Dans cette population, ils ont trouvé 18.8 % de PTSD, 57.2 % de naissances traumatiques. (non représentatif de la population générales car la cible était des femmes qui estimaient avoir eu un accouchement difficile ou traumatique)
- identifier la présence ou non de ’hotspot’ (les moments de détresse extrême, survenant pendant un événement traumatisant) et combien ; 67% des femmes avaient eu au moins un hotspot, médiane 3 hotspots
- décrire le pire de ces hotspots ;
- décrire les “cognitions et émotions“ pendant les hotspots en utilisant une échelle (par exemple: “panique“, “détaché comme dans un rêve“... grille complète page 21, échelle 0 “pas du tout“ à 3 “extrêmement)
Les chercheurs ont classifié ces hotspot selon leur contenu thématique à partir de leur description: relations interpersonnelles (être ignoré, manque de support, mauvaise communication, abandon, pression); événements concernant le bébé (problème avec le bébé, séparation); événements obstétricaux et douleur
Conclusions: le risque de stress post-traumatique est plus élevé :
- si les hotspots concernent des difficultés interpersonnelles ou des complications obstétricales, par comparaison à des complications avec le bébé (catégorisation de la description);
- si les hotspots comportent de la peur et de la perte de contrôle, ou des dissociations intrapartum (échelle des cognition et sentiments

Etude sur prévalence et facteurs de risques d’état de stress-posttraumtique après l’accouchement en France: 5% à un mois, 3% à 9
mois.
L’étude s’intéresse aux facteurs prédictifs, parmi lesquels n’apparaît pas l’idée d’attitudes et comportement maltraitants de
la part des soignants. Ce qui apparaît dans cette étude comment dans de nombreuses autres: le sentiment de perte de contrôle
pendant l’accouchement.

Dans les conclusions: “les professionnels de périnatalité ont une responsabilité de plus: fournir une information claire et
