OBJECTIF: Évaluer par une revue de la littérature l’intérêt du massage périnéal ante-partum pour réduire le nombre de déchirures périnéales ou d’épisiotomies.

MATERIEL ET METHODES: Recherche des essais cliniques randomisés en anglais ou français sur le sujet en utilisant la banque de données du Medline ou du Cochrane Library. Les années de recherche ont été de 1966 à Novembre 2000, en utilisant les mots clés suivants : « perineum », « massage », « perineum injuries », « randomized controlled trial ».

RESULTATS: 4 essais cliniques randomisés ont été finalement sélectionnés. La définition concernant les critères de jugement retenus, les critères de sélection et d’exclusion des femmes varient selon les auteurs. Dans les plus grosses études, le massage périnéal semble réduire le nombre des déchirures du périnée surtout chez la primipare puisque Labrecque et al. en 1999, retrouvaient un OR de 0,56 ; IC 95 % : 0,40-0,79, et une augmentation des périnées intacts chez ces femmes (OR = 1,79 ; IC 95 % 1,27-2,52) ; et Shipman et al. en 1997 notaient aussi, mais seulement chez les femmes de plus de 30 ans une augmentation des périnées intacts en cas de massages (OR = 1,93 ; IC 95 % 1,08-3,48), et dans une régression logistique tenant compte de l’âge de la mère et du poids de naissance de l’enfant, ils retrouvaient une réduction des épisiotomies et des déchirures périnéales graves en cas de massages du périnée (p = 0,02).

CONCLUSION: Le massage périnéal ante partum semble pertinent mais d’autres études sont nécessaires pour apprécier l’effet de cette intervention sur les déchirures graves du périnée et l’acceptabilité de la méthode par les femmes.

Abstract (English)  :

OBJECTIVE: To assess the effectiveness of ante partum perineal massage to reduce the number of perineal injuries and episiotomies through a survey of the existing literature.

MATERIEL AND METHODS: A search both in English and French on randomized clinical trials using the Medline and Cochrane Library databases. The key words: "Perineum", "massage", "perineum injuries", "randomized controlled trial" were selected from the years 1966 to November 2000.

RESULTS: Four randomized controlled trials were found. The definition of the selected issues, as well as the included and excluded criteria varied according to the authors. Perineal massages seemed to reduce the occurrence of perineal injuries and episiotomies, mostly among primipara: Labrecque et al. in 1999, noted an OR of 0.56; 95% CI: 0.61-1.31 and at the opposite an increased rate of intact perineum in the massage group (OR = 1.79; 95% CI 1.27-2.52]; and Shipman et al. in 1997 stressed among women of > or = 30 years old an augmentation of intact perineum in the intervention group (OR = 1.93; 95% CI 1.08-3.48), and in the logistic regression taking into account age and birth weight they found a reduction of episiotomies and important perineal injuries (p = 0.02).

CONCLUSION: Ante partum perineal massages would seem valid but further studies would be necessary to evaluate the utility of this intervention in the avoidance of serious perineal injuries and the women’s satisfaction.
