Une étude suédoise par entretiens : l’évaluation parentale des risques lors des naissances à domicile.

OBJECTIFS : décrire l’évaluation du risque de la naissance à domicile par les parents.

METHODE : entretiens conduits à l’aide de questionnaires semi-directifs. Les données ont été analysées en utilisant une approche phénoménologique.

CADRE : exercice libéral de la profession de sages-femme en Suède.

PARTICIPANTS : cinq couples qui avaient programmé une naissance à domicile.

RESULTATS : Les parents avaient une confiance fondamentale dans le fait que la naissance allait se dérouler sans complications, et ils en ont ressenti le sens profond durant l’événement lui-même. Les risques étaient considérés comme une part d’un phénoméne complexe qui n’était pas limité à l’acccouchement à domicile. Cette attitude a semblé faire partie d’un mode de vie qui était lié á la manière dont les risques rencontrés durant la naissance étaient traités.
Cinq catégories ont été identifiées pour contrebalancer les risques de possibles complications : (1) confiance dans les capacités de la femme à donner naissance, (2) confiance en l’intuition, (3) confiance dans la sage-femme, (4) confiance dans la relation, et (5) préparation physique et intellectuelle.

CONCLUSIONS CLEFS : Bien que les parents soient conscients du risque de complications durant l’accouchement, la confiance fondamentale dans la capacité intrinsèque de la femme à donner naissance était centrale dans la décision de choisir un accouchement à la maison. Une grande importance était accordée aux effets positifs attendus d’une naissance à domicile.

CONSEQUENCE POUR LA PRATIQUE : mieux connaître évaluation du risque que font les parents peut promouvoir une meilleure compréhension de la manière avec laquelle les parents en devenir ressentent les risques associés à une naissance á domicile.

Abstract (English)  :

OBJECTIVE: to describe home-birth risk assessment by parents.

DESIGN: interviews using a semi-structured interview guide. Data were analysed using a phenomenological approach.

SETTING: independent midwifery practices in Sweden.

PARTICIPANTS: five couples who had had planned home births.

FINDINGS: the parents had a fundamental trust that the birth would take place without complications, and they experienced meaningfulness in the event itself. Risks were considered to be part of a complex phenomenon that was not limited to births at home. This attitude seems to be part of a lifestyle that has a bearing on how risks experienced during the birth were handled. Five categories were identified as counterbalancing the risk of possible complications: (1) trust in the woman’s ability to give birth; (2) trust in intuition; (3) confidence in the midwife; (4) confidence in the relationship; and (5) physical and intellectual preparation.

KEY CONCLUSIONS: although the parents were conscious of the risk of complications during childbirth, a fundamental trust in the woman’s independent ability to give birth was central to the decision to choose a home birth. Importance was attached to the expected positive effects of having the birth at home.

IMPLICATIONS FOR PRACTICE: knowledge of parents’ assessment can promote an increased understanding of how parents-to-be experience the risks associated with home birth.
