Résultats des naissances prévues à domicile dans l’Etat de Washington, USA, 1989-1996.

Objectif : Déterminer si il y avait une différence entre les naissances prévues à domicile et les naissances prévues à l’hôpital dans l’état de Washington, au regard de certaines conséquences défavorables à l’enfant (mort néonatale, faible score d’Apgar, nécessité d’une aide à la respiration) et à la mère (travail prolongé, saignement post-partum).

Méthode : Nous avons étudié le registre des naissances de l’état de Washington de 1989 à 1996 pour les naissances sans complication d’enfants issus d’une grossesse unique d’au moins 34 semaines de gestation qui sont nés soit à la maison avec un professionnel de santé (N=5854) soit qui ont été transférés dans un centre médical après un essai de naissance à domicile (N=279). Ces naissances prévues à domicile ont été comparées aux naissances d’enfants issus de grossesses uniques prévues à l’hôpital (N=10 593) au cours de la même période.

Résultats : Les enfants dont la naissance était prévue au domicile présentaient un risque accru de mort néonatale (risque relatif ajusté [RR] 1,99 95% intervalle de confiance [CI] 1,06 3,73) et un score d’Apgar non supérieur 3 à 5 minutes (RR 2,31 95% CI 1,29 4.16). Les relations restent inchangées avec une analyse restreinte aux grossesses d’au moins 37 semaines de gestation. Parmi les nullipares uniquement, ces naissances étaient aussi associées un risque accru de travail prolongé (RR 1,73 95% CI 1,28 2.34) et de saignement post-partum (RR 2,76 95% CI 1,74 4.36).

Conclusion : Cette étude suggère que les naissances prévues à domicile dans l’état de Washington entre 1998 et 1996 présentaient un risque accru pour la mère et pour l’enfant par rapport aux naissances prévues à l’hôpital.

Abstract (English)  :

OBJECTIVE: To determine whether there was a difference between planned home births and planned hospital births in Washington State with regard to certain adverse infant outcomes (neonatal death, low Apgar score, need for ventilator support) and maternal outcomes (prolonged labor, postpartum bleeding). METHODS: We examined birth registry information from Washington State during 1989-1996 on uncomplicated singleton pregnancies of at least 34 weeks’ gestation that either were delivered at home by a health professional (N = 5854) or were transferred to medical facilities after attempted delivery at home (N = 279). These intended home births were compared with births of singletons planned to be born in hospitals (N = 10,593) during the same years. RESULTS: Infants of planned home deliveries were at increased risk of neonatal death (adjusted relative risk [RR] 1.99, 95% confidence interval [CI] 1.06, 3.73), and Apgar score no higher than 3 at 5 minutes (RR 2.31, 95% CI 1.29, 4.16). These same relationships remained when the analysis was restricted to pregnancies of at least 37 weeks’ gestation. Among nulliparous women only, these deliveries also were associated with an increased risk of prolonged labor (RR 1.73, 95% CI 1.28, 2.34) and postpartum bleeding (RR 2.76, 95% CI 1.74, 4.36). CONCLUSION: This study suggests that planned home births in Washington State during 1989-1996 had greater infant and maternal risks than did hospital births.
