Parmi les issues possibles, indésirées et inattendues, de la maternité, figure en
bonne place l’état de stress post-traumatique (ESPT), avec la dépression postpartum (DPP) et la psychose. Bien que dans ce contexte son origine soit tres différente de celle de l’ESPT du par exemple à des combats en tant de guerre, on retrouve le même ensemble basique de déclencheurs, réactions, et comportements.

Les conséquences et l’impact sur la qualité de vie des mères sont manifestement
tres différentes de celles de la DPP, bien que l’ESPT puisse y conduire. Cependant, il est peu vraisemblable que la guérison soit spontanée comme c’est souvent le cas pour la DPP. Ainsi l’ESPT nécessite un traitement actif en suivant l’option que l’on estimera la plus adaptée. En outre, sachant que les circonstances donnant lieu à l’ESPT sont souvent des procédures médicales standards, les médecins doivent consciemment casser les valeurs et standards de leur propre discipline lorsqu’ils soignent ces patients.

l’ESPT est un trouble peu reconnu qui, même lorsqu’il l’est, est souvent sous-traité. Se produisant dans le contexte d’un accouchement, il peut être masqué par d’autres entités tels que la dépression, l’anxiété, la somatisation, l’abus de drogues, des troubles de l’alimentation, et même la psychose ou un esprit litigieux. Pour en réduire la morbidité psychiatrique, physique et interpersonnelle, il est nécessaire de reconnaître ce trouble, d’appliquer des mesures préventives quand c’est possible, et de diminuer la détresse occasionnée par une intervention précoce et effective.

Lors ce symposium, nous donnerons la parole à des femmes et des familles qui ont personnellement vécu un ESPT, puis décrirons aux participants les facteurs de risque, les possibilités de traitement, et quelques aspects des théories en biochimie sur ce trouble.

Abstract (English)  :

Recent research has shown that PTSD stands alongside Post Natal Depression (PND) and Psychosis as one of the potential unwanted and unexpected legacies of childbearing. Although its origins in this context are very different from PTSD arising for example from combat scenarios, the same basic set of triggers, reactions and attitudes is evident.

The outcomes and effects on the mothers’ quality of life differ demonstrably from those of PND, although PTSD can lead for that. However, recovery is not likely to be spontaneous as is often the case for PND, and hence PTSD requires active treatment by whatever option is deemed most suitable. Moreover, because the circumstances giving rise to PTSD are often standard medical procedure, physicians must consciously set aside the values and standards of their own discipline when treating sufferers.

PTSD is an under recognised disorder which, even when recognised is often under treated. Occurring in the context of childbirth, it may masquerade as other entities including depression, anxiety, somatisation, substance abuse, eating disorders and even psychosis or litigation. In order to minimise morbidity, both psychiatric, physical and interpersonal, it is necessary to recognise this disorder, apply preventative measures where possible, and minimise the distress caused by early and effective intervention.

As well as hearing from the women and families who have had personal experience of PTSD, this symposium will take participants through risk factors, prevention strategies, treatment options and some of the biochemical theories about the disorder.
