Dans un souci d’optimisation de la réponse médicale concernant les urgences chez les femmes enceintes, le SAMU Régional de Lille a mené une étude prospective sur le versant « régulation médicale », mais également sur le versant « intervention ». L’étude UPREFE (Urgences PREhospitalières et Femmes Enceintes) a associé les médecins régulateurs du SAMU et les équipes des différents SMUR du département du Nord. Les accouchements à domicile représentant un sous-groupe de patientes de l’étude, ils ont été analysés depuis l’appel au Centre 15 jusqu’au devenir de la grossesse. Mille vingt-sept patientes ont été incluses dans cette étude. Parmi elles, 716 ont eu recours au SAMU Centre 15 pour accouchement imminent : pour ces dernières, les équipes médicales des SMUR du département du Nord ont réalisé 135 accouchements extrahospitaliers. Parmi ces 135 patientes, 33 ont accouché à domicile avec un score de Malinas 1 inférieur à 5, ce qui conduit à émettre l’hypothèse que ce score de Malinas, bien que très précieux pour les médecins régulateurs des SAMU, ne permet pas, à lui seul, d’évaluer l’imminence d’une naissance. Ces constatations permettent d’argumenter l’intérêt d’une prise en charge dynamique des appels pour accouchements à domicile par les SAMU, mais doivent également inciter à optimiser les politiques de périnatalité afin d’éviter ces naissances à domicile non programmées.

Abstract (English)  :

A prospective study was conducted with the aim of providing the most suitable response for pregnant women calling the Lille Emergency Medical Units (SAMU) Center 15 for immediate delivery. The study involved SAMU physicians and other Emergency care teams in the North of France. Unattended home births were analyzed involving a total of 1 027 women, including 716 calls to center 15 concerned unattended births and 135 prehospital deliveries attended by emergency medical care units. The Malinas score was below 5 for 33 of these 135 "home" deliveries. This led us to the hypothesis that the Malinas score is insufficient for making medical decisions and cannot be used alone to determine the eminence of delivery by rather should be considered as an associated tool for the physician coordinating Emergency medical teams.
