Etude de cohorte sur les accouchements prévus domicile ou à l’hôpital en Australie Occidentale 1981-1987

Objet : évaluation des pratiques comparant les accouchements planifiés à domicile et les accouchements planifiés à l’hôpital.
Conception: analyse rétrospective d’une cohorte ayant planifié un accouchement à domicile comparé à une cohorte similaire d’accouchement à l’hôpital.
Lieu : Australie occidentale (WA)
Panel : toutes les femmes (n=976) ayant planifié un accouchement à domicile entre 1981 et 1987 et 2928 femmes au profil similaire ayant planifié un accouchement à l’hôpital – grossesse unique exclusivement.

Mesures et résultats : Les femmes dans le groupe des accouchements à domicile ont eu un travail plus long, étaient moins susceptibles d’avoir un travail déclenché ou d’avoir un accouchement opératoire (note de traduction: “operative delivery“ qui doit inclure extractions instrumentales et césariennes). Elles ont généralement eu moins de complications durant le travail, mais ont eu plus d’hémorragies du post-partum et de rétention placentaire. Les bébés nés à la maison avaient un poids supérieur et sont plus souvent nés après terme. Ils ont eu moins de score Apgar inférieur à 8 à 5 minutes, ont moins souvent mis plus d’une minute à respirer seuls et ont eu moins souvent besoin de réanimation. Le ratio brut des cotes (crude odds ratio) pour la mortalité périnatale des accouchements planifiés à la maison était 1.25 (95% CI 0.44 – 3.55) La mortalité postnatale était plus commune à l’hôpital. Les accouchements planifiés à domicile ont généralement connu moins d’interventions postnatales, avec une morbidité maternelle et de l’enfant inférieure, à l’exception des complications de 3è degré (third stage complications). Bien que non significative, une augmentation de la mortalité périnatale a été observée dans d’autres études australiennes sur l’accouchement à domicile et les analyses doivent êtes étendues.

Conclusions : Les accouchements planifiés à domicile en Australie Occidentale semblent être associés à une morbidité généralement inférieure, pour la mère et l’enfant, et à de moindres interventions médicales qu’à l’hôpital.

Implications pour les pratiques : Il n’a pas pu être déterminé par cette étude si les différences observées d’interventions médicales et de morbidité avaient un impact ou non sur la petite et non-significative augmentation de mortalité périnatale. Des évaluations plus approfondies de l’accouchement à domicile et de ses résultats devront être menées.

Abstract (English)  :

OBJECTIVE: to evaluate practice comparing planned home birth with planned hospital birth DESIGN: a retrospective analysis of a cohort who had planned to have a home birth compared with a matched hospital birth group
SETTING: Western Australia (WA)
PARTICIPANTS: all women (N = 976) who ’booked’ to have a home birth 1981-1987 and 2928 matched women who had a planned hospital birth (singleton births only).

MEASUREMENTS AND FINDINGS: women in the home birth group had a longer labour, were less likely to have had labour induced or to have had any sort of operative delivery. They were less likely overall to have had complications of labour, but more likely to have had a postpartum haemorrhage and more likely to have had a retained placenta. Babies in the home birth group were heavier and more likely to be post-term. They were less likely to have had an Apgar score below 8 at 5 minutes, to have taken more than 1 minute to establish respiration or to have received resuscitation. The crude odds ratio for planned home births for perinatal mortality was 1.25 (95% CI 0.44-3.55). Postneonatal mortality was more common in the hospital group. Planned home births were generally associated with less intervention than hospital births and with less maternal and neonatal morbidity, with the exception of third stage complications. Although not significant, the increase in perinatal mortality has been observed in other Australian studies of home births and requires continuing evaluation. KEY

CONCLUSIONS: Planned home births in WA appear to be associated with less overall maternal and neonatal morbidity and less intervention than hospital births.

IMPLICATIONS FOR PRACTICE: whether these observed differences in intervention and morbidity have any relationship to the small, non-significant increase in perinatal mortality could not be determined in this study. Continuing evaluation of home birth practice and outcome is essential.
