Deux groupes d’experts récents ont conclu que la preuve de la sécurité relative du domicile à la maison par rapport à l’accouchement à l’hôpital ne justifiait pas une recommandation générale pour l’accouchement à l’hôpital. Bien que la preuve ait été contestée, l’innocuité ne peut être testée dans le cadre d’un essai contrôlé randomisé en raison des chiffres requis et devra être évaluée aussi bien que possible par d’autres méthodes. Néanmoins, une autre incertitude sous-jacente au débat concerne l’effet du lieu d’accouchement sur les résultats psychologiques et l’alimentation du nourrisson. Les hypothèses selon lesquelles la naissance à domicile entraîne moins d’anxiété et des taux d’allaitement plus élevés sont vérifiables dans le cadre d’un essai contrôlé randomisé de taille modeste. Par exemple, si les taux d’allaitement au sein étaient de 50% à l’hôpital, un essai sur 100 femmes par groupe aurait le pouvoir d’exclure une augmentation de 20% du nombre de femmes ayant eu un accouchement à domicile (alpha 0,05). Cependant, la plupart des experts que nous avons consultés nous ont informés que les femmes refuseraient d’être affectées au hasard à un accouchement à domicile ou à l’hôpital dans un tel essai. Nous avons donc réalisé une petite étude de faisabilité.

Abstract (English)  :

Two recent expert groups have concluded that the evidence for the relative safety of home compared with hospital delivery does not justify a general recommendation for hospital delivery. Although the evidence has been disputed, safety is not testable in a randomised controlled trial because of the numbers required and will have to be assessed as well as possible by other methods. Nevertheless, another uncertainty underlying the debate is the effect of place of delivery on psychological outcomes and infant feeding. The hypotheses that home birth results in less anxiety and higher rates of breast feeding are testable in a randomised controlled trial of only modest size. For example, if breast feeding rates were 50% in hospital a trial of 100 women per group would have the power to exclude a 20% increase among women offered home delivery (alpha 0.05). Most experts we consulted told us, however, that women would decline being randomly allocated to home or hospital delivery in such a trial, and so we performed a small feasibility study.
